About the Role
New Horizon provides the only youth-specific street outreach service to young people facing homelessness in London. This role supports young people by placing them into appropriate and safe accommodation and encouraging them to access the day centre's services. The position involves regular street outreach shifts and supporting young people at the day centre, contributing to a best practice approach for supporting young rough sleepers.
Responsibilities
- Contribute to the delivery of the NHYC street outreach service, enabling young people who are street homeless to move on to independence.
- Build and maintain a network of external partnerships with services supporting those impacted by rough sleeping.
- Enable the delivery of a trauma-informed service to young people.
- Support the delivery of NHYC’s rough sleeping prevention strategy and best practice youth-specific outreach services.
- Provide comprehensive support through 1-1 and small group work with young people sleeping rough.
- Work within the Rough Sleeping team to ensure young people receive a high-quality service from registration to move-on.
- Carry out at least two street-based outreach shifts each week, including early morning and late evening shifts.
- Conduct housing assessments with young people and refer them into appropriate accommodation.
- Manage a transitional caseload of young people, assisting them in moving forward and accessing appropriate services.
- Collaborate with the Outreach Team and other external teams, including mental health and substance misuse services.
- Provide expert coaching and guidance to young people, monitoring and evaluating their progress.
- Complete registrations, outcomes assessments, risk assessments, and action plans with young people.
- Record all relevant work on the New Horizon database system and Chain database.
- Ensure risk management and assessment procedures are followed.
- Deliver advocacy and referrals to appropriate services, including internal referrals.
- Contribute to the management and development of the team and day centre, including attending daily handovers and ensuring Health and Safety issues are addressed.
- Attend Reflective Practice and team meetings.
- Take a hands-on approach to working with young people.
- Participate in the training and supervision of new staff, volunteers, students, and locum workers.
- Actively promote a learning and development environment.
- Proactively liaise, communicate, and negotiate with internal specialists and external agencies.
- Seek out and respond to opportunities with external partners for resources, funding, or services.
- Promote and represent New Horizon Youth Centre at relevant forums and meetings.
- Produce and maintain accurate and useful information in various formats to promote effective service delivery and evaluation.
- Produce written reports for team managers, service providers, and funders.
- Input and extract information from client monitoring systems and other database systems.
- Perform self-servicing in day-to-day administration and follow team and New Horizon’s administrative procedures.
- Proactively review and evaluate own performance, identifying and acting upon areas for improvement.
- Continuously review own working practices in line with feedback and current best practice.
- Take a proactive approach to supervision and attend clinical supervision, weekly team meetings, and line management supervision.
- Review and evaluate own performance to identify strengths and areas for development.
- Undertake development and training opportunities and ensure maximum benefit through review, reflection, and practice.
- Contribute positively and constructively to the development of the team, service, and Centre.
- Cover for other team members as necessary.
- Follow New Horizon’s policies, procedures, and performance expectations.
- Undertake any other duties compatible with the level and nature of the post as required.
Requirements
- Ability to work effectively with young people facing homelessness to identify personal goals and support mechanisms needed for change.
- Skills, ability, and confidence to carry out street outreach shifts at least twice weekly, including late evenings and early morning shifts across London.
- Experience of creating and delivering work both 1-1 and in groups which meet the needs of young people.
- Experience of using IT systems (including knowledge of CHAIN and other databases) to support work goals.
- Ability to multitask in a pressured environment.
- Ability to build and maintain effective working relationships with a diverse range of internal stakeholders and external agencies.
- Ability to maintain enthusiasm for a high level of contact with young people and a genuine interest, passion, and commitment to help them access opportunities leading to independence.
- Ability to hold and manage a transitional caseload of young people with multiple complex needs, supporting them into accommodation outcomes and linking them with specialist support services.
- Understanding of challenging behaviour that young people might demonstrate and ability to deliver strategies for dealing with such behaviour.
- High level of knowledge of health and safety issues in a working environment, particularly in the context of street-based outreach.
- Experience and knowledge around safeguarding policy and procedures for children and young people.
- Working knowledge of and commitment to Equity, Diversity, and Inclusion as it applies to a supportive service and in the workplace.
- Willingness and ability to work outside of normal office hours, including early morning and late evening shifts, and over the Christmas period.
- Willingness to work flexibly in response to changing organisational requirements.
- Willingness to travel across London for street outreach shifts and partnership building.
- Thorough and up-to-date working knowledge of Local and National Government policies and procedures in relation to the client group.
- Thorough and up-to-date working knowledge of relevant organisations and their role/responsibilities in providing support to vulnerable young people.
- Knowledge of the CHAIN database and street-based outreach services.
- Knowledge of housing options for young people sleeping rough.
- Ability to create and maintain external partnerships to support organisational aims and objectives, including representation of the organisation at external meetings and events.
- Knowledge of working to performance indicators, including monitoring and reporting on service outputs/outcomes, client tracking, analysing performance information, and identifying corrective action.

Work Type

Salary/Compensations
- Starting salary: £32,136.00
- Salary scale: AP26 – AP30 (£32,136.00-£35,778.00)
