About the Role
Professional Care Medical Practice P.C. is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional within the medical department at the Monroe County Jail in Rochester, NY. This role focuses on providing clinical monitoring, coordination, and implementation of the mental health program according to NCCHC standards, acting as a liaison between the medical unit and facility staff.
Responsibilities
- Conduct clinical assessments, including documentation of findings, case conceptualization, diagnosis, and treatment plans.
- Make referrals to psychiatry when appropriate.
- Conduct risk assessments using structured interviews and tools.
- Manage inmates at risk for suicide and/or self-harm according to established protocols.
- Serve as the PCM mental health liaison for external mental health services.
- Facilitate the admission and placement of inmates/patients to forensic hospitals.
- Monitor all provided mental health services.
- Ensure clinical services meet NCCHC and accepted psychiatric/psychological standards.
- Participate as a member of the Quality Assurance Committee.
- Develop and conduct in-service education programs.
- Collect data and prepare monthly performance reports.
Requirements
- Must be licensed as a LMHC, LMSW, or LCSW in New York State.
- Must hold a Master’s degree or greater in Social Work, Psychology, or a related Human Service field with a clinical concentration.
- Minimum of two years of experience and familiarity with substance abuse/MAT counseling preferred.
- Must hold a valid driver’s license.
- Must have reliable transportation.

Salary/Compensations
- $30.00- 40.00/hr
Benefits
- Part Time PTO package provided and accrued at the rate of 1.0 hour for every 30.0 hours of straight time worked.
- Maximum PTO accrual of 64.0 hours.
