About the Role
Community Healthcare Network is seeking a Full-time Family Nurse Practitioner for our Williamsburg Health Center. This role involves providing evidence-based Primary Care to patients as an integral part of a collaborative care team.
Responsibilities
- Providing evidence-based Primary Care to patients.
- Being an integral part of the collaborative care team, which includes nursing staff and other services such as Health Education, Nutrition, Social Work, and Behavioral Health.
Requirements
- Valid Family Nurse Practitioner N.Y.S. License to deliver medical care.
- At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
