About the Role

Leads the care management team in administrative activities, including member outreach, call handling, and service scheduling. Acts as a liaison for members, providers, and staff to resolve care plan-related issues and maintain accurate member records according to regulatory guidelines.

Responsibilities

  • Leads and guides member outreach via phone to support care plan next steps, community or health plan resources, and scheduling.
  • Provides ongoing support to members to connect them with health plan and community resources.
  • Serves as front-line support for complex member and/or provider inquiries, explaining care plan procedures.
  • Works with the care management team to identify and recommend process improvements for administrative care management.
  • Performs team member onboarding and administrative duties, including sending welcome letters and educational materials.
  • Works with the care management team on escalating complex requests and inquiries to management.
  • Documents and maintains all non-clinical member records in accordance with state and regulatory requirements.
  • Contributes to the development of education and training programs.
  • Performs other duties or responsibilities as assigned to meet business needs.
  • Complies with all policies and standards.

Requirements

  • Requires a High School diploma or GED.
  • Requires 4+ years of related experience.
  • Requires a Texas DFPS background check.

Skills

  • Strong documentation skills with demonstrated audit performance in TruCare Cloud.
  • Proficiency with Microsoft Office Suite, including Excel, OneNote, PowerPoint, and Outlook.
  • Excellent problem-solving, organization, time management, and communication skills.
  • Ability to adapt quickly to changing business priorities.
  • Ability to communicate data, findings, and recommendations clearly to cross-functional stakeholders.
