About the Role
Participates in the application of the nursing process to an assigned group of patients, demonstrating the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse.
Responsibilities
- Applies the nursing process to assigned patients.
- Demonstrates scientific knowledge, judgment, and technical skills of an LPN.
- Performs job duties under the supervision of a Registered Professional Nurse.
- Provides skilled nursing care tasks and procedures including vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs, urinary catheter insertion/care, and simple dressing removal.
- Administers IV fluids upon completion of annual competency training.
- Monitors patients receiving blood products and TPN under RN direction.
- Makes recommendations for the nursing process.
- Participates in planning and administers direct patient care, including ADLs.
- Performs point of care testing, including blood glucose.
- Performs nursing procedures as defined in the nursing procedure manual.
- Administers medications via all routes except IV push/bolus (excluding saline/heparin flush) or central line; does not administer chemotherapy or immunotherapy.
- Administers subsequent doses of IV drugs after RN administers the first dose and assesses for side effects.
- Participates in hemodialysis-related tasks in chronic outpatient hemodialysis settings under direct RN supervision and after completing approved training, as permitted by NYSED.
- Assists in patient assessment, including pain management, after initial RN assessment.
- Carries out medical and nursing orders; cannot accept verbal orders or carry out RN standing orders.
- Makes, reports, and records patient observations within the EMR.
- Observes and reports changes in patient condition to the appropriate person.
- Participates in health teaching to patients and families following RN-developed teaching plans.
- Establishes an environment that promotes optimum physical and emotional comfort.
- Performs hourly care checks and supports the patient experience.
- Organizes necessary supplies and equipment for the plan of care.
- Contributes specified data to patients’ medical records.
- Provides care based on age-specific needs and criteria (Neonate/Infant, Pediatric, Adolescent, Adult, Geriatric).
- Demonstrates awareness of Nursing Department goals.
- Supports the patient experience to achieve safe and quality care.
- Adapts to changing departmental needs.
- Manages time effectively to meet unit/department needs.
- Interprets hospital policies and regulatory standards of care.
- Recommends changes in nursing practice and policy.
- Participates in the Nursing Department Quality Improvement Program.
- Maintains patient/employee confidentiality.
- Participates in the development of other staff members.
- Meets regulatory, licensure, and annual health assessment requirements.
- Participates in patient care conferences.
- Maintains nursing care standards through workshops, in-service programs, and staff meetings.
- Maintains competence through continuing education.
- Demonstrates a professional, courteous, and respectful attitude with patients, families, and staff.
- Displays courtesy, professionalism, and respect during interactions with all members of the hospital staff and extended community.
- Is responsible to the leadership person of the unit.
- Provides direction, assistance, and support to auxiliary staff.
- Participates in interdepartmental and intergroup communication for continuity of patient care.
- Participates in departmental committees as assigned.
- Adheres to MSHS legal and ethical standards.
- May be required to perform other duties as assigned.
- Administers bolus of medications by IV push in a chronic outpatient hemodialysis treatment center (excluding first dose).
- Identifies and sets up supplies for accessing a CVAD.
- Initiates treatment with a CVAD.
- Draws blood specimens from a CVAD.
- Performs CVAD dressing change and exit site care.
- Monitors and adjusts flow rates of central venous catheter access device/line.
- Intervenes in case of catheter/blood line disconnection or other CVAD complications.
- Provides CVAD care at the end of dialysis treatment.
Requirements
- High School Diploma or GED
- Licensed and currently registered or otherwise duly authorized to practice as a Licensed Practical Nurse in New York State.
- Basic Life Saver (BLS) certification from AHA or Red Cross.
