About the Role
The Mobile Senior Services Coordinator plays a proactive role in assisting active and at-risk seniors in accessing and receiving social services to enable them to remain independent and well-supported at designated Housing sites. This position serves seniors and adults with disabilities in San Francisco by providing social services, wellness activities, referrals, and support. The role also requires direct administrative and operational support, overseeing volunteers and instructors, and demonstrating initiative, strong organization, outreach, follow-up, and follow-through skills.
Responsibilities
- Complete intakes and assessments for incoming participants for the Active Senior Program and complete data entry using the DAAS GetCare system.
- Complete participant charting, monthly reports, and other documentation as required by BHNC and funding sources.
- Provide direct short-term social services such as completing applications, translations, reviewing correspondence, information and referrals, and arranging for care for participants.
- Evaluate client needs, follow-up, and document progress notes to ensure quality service delivery.
- Provide administrative support during programming for activities, field trips, newsletters, and translations.
- Provide outreach to frail, at-risk elders and disabled adults with low incomes at the sites to encourage participation.
- Schedule and coordinate meetings with the advisory board and assist in outreach to increase board membership.
- Assist in the implementation of schedules and duties for instructors (volunteer or paid).
- Work with property management to resolve or assist clients with needs at the housing sites.
- Maintain comprehensive records on the selection, training, and activities of volunteers.
- Provide recommendations for program improvements and alterations.
- Respond to public inquiries to provide information on volunteer programs.
- Attend all BHNC meetings, events, and activities as requested, including monthly Senior Staff, BHNC Staff, and advisory committee meetings.
- Perform other duties as assigned.
Requirements
- Must be 21 years or older.
- Must have a valid Driver's License.
Skills
- Proficient computer skills.
- Strong verbal and written communication ability.
- Experience with Microsoft Office Programs (Word, Excel, PowerPoint).
- Experience with Google Suite (Docs, Sheets, Slides, Drive, Calendar, Forms).
- Experience with Slack preferred.
- Experience with Asana preferred.
- Experience or desire to learn data entry into DAS GetCare database.
- Ability to work in an active setting with staff and volunteers.
- Ability to work a schedule from 9:00 am - 5:00 pm, Monday - Friday.
- Ability to provide excellent customer service.

- BA degree, or AA/AS or 3 years of social services, community experience, preferably working with older adults and adults with disabilities.
