About the Role

The PROS Case Associate provides supportive and psychiatric rehabilitation services to program participants, aiming to help them achieve goals related to housing, work, education, and socialization, and to integrate into the community.

Responsibilities

  • Assessment, service planning, and provision of services for assigned caseload under social worker supervision.
  • Provide PROS services through individual meetings with participants.
  • Record progress notes for client records and groups.
  • Make appropriate referrals to other agencies as part of discharge plans.
  • Plan and facilitate wellness, skills-building, and rehabilitation readiness groups under social worker supervision.
  • Participate in program activities to help individuals reach their goals.
  • Provide information, advocacy, support, and outreach/engagement services to clients.
  • Perform clinically-focused case management activities, including linkage, referrals, coordination between service providers, and family involvement in treatment.
  • Provide structure to clients, evaluate and assess independent living skills, and offer guidance in these areas.
  • Participate in staff meetings and trainings.
  • Prepare materials for and participate in treatment conferences, including intake, assessment, discharge, and follow-up.
  • Communicate in a timely and responsible manner with the treatment team for appropriate client care.
  • Perform other duties as assigned.

Requirements

  • BA or BSW Degree.
  • Some experience in a community or social service group.
  • Experience working with adults who are mentally ill strongly preferred.
  • Good writing skills.
  • Minimum of two years of related work experience, preferably working with adults with mental illness.
  • General knowledge in job development and job securing is strongly desired.
  • Excellent organizational skills, leadership, teamwork, and communication skills.
  • Related vocational experience is preferred for vocational-focused positions.
  • Basic computer skills, including Microsoft Outlook and Word.
  • Experience working in an electronic medical record preferred.
  • Ability to read paper and electronic documents.
  • Ability to perform significant data entry into various computer programs.
  • Ability to sit (90% of the time) and stand (10% of the time).
