About the Role
The Housing Stability Advocate/Case Manager is a specialized role designed to drastically decrease homelessness recidivism in Albany. Utilizing an enhanced Critical Time Intervention (CTI) model, this position provides the necessary support for households transitioning from crisis to independence, bridging the support gap as they move into permanent housing.
Responsibilities
- Complete comprehensive intake and ongoing assessments.
- Establish Service Plans with concrete, time-bound goals.
- Ensure timely, accurate documentation of service delivery in the electronic health record.
- Provide 1-on-1 teaching, coaching, and rehabilitative counseling to help households build skills needed to maintain an apartment.
- Serve as a household advocate by building and maintaining positive landlord relationships.
- Provide conflict mediation with landlords while upholding professional boundaries.
- Coordinate service plan delivery through ongoing communication with area providers, families, and community networks.
- Connect households with mental health, education, employment, and non-healthcare resources.
- Engage directly with youth and households in their homes and community settings using a positive, strength-based, person-centered, and trauma-informed approach.
- Intervene in crises using Therapeutic Crisis Intervention (TCI) strategies.
- Assist with conflict resolution.
- Travel locally to provide in-home/community services.
- Transport clients to appointments.
- Attend all mandated agency trainings assigned by the supervisor.
- Provide additional support and rehabilitative services based on new contracts, changing regulations, and other position-related duties.
Requirements
- High School Diploma or equivalent (GED/ TASC) required; College degree highly preferred.
- A minimum of one-year previous experience working in homeless services, or human services.
- Flexibility schedule availability, including the ability to work outside regularly scheduled hours when necessary.
- Demonstrated ability to work collaboratively within a team and build strong rapport with clients.
- Clean and Valid New York State Driver’s License (actively licensed for at least 1 year) with reliable transportation.
- Excellent organizational, planning, time management, and written communication skills.
- Strong computer skills, including proficiency in Microsoft Office Suite, Google Workspace, and secure internet practices.
- Warm, empathetic, and trauma-informed approach, with the ability to provide compassionate care.
- High ethical standards, exercising sound independent judgment, discretion, and strict client confidentiality.
- Reliable, self-motivated, and adaptable to fast-paced, day-to-day operational changes.
- Must be at least 21 years old at time of hire to meet insurance requirements for authorized drivers.
- Must be able to maintain insurability throughout employment.
