About the Role

Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, organized around five pillars of healthy aging. This role involves leading longevity days at partner senior living communities, including educational talks and one-on-one visits with residents.

Responsibilities

  • Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment.
  • Evaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriate.
  • Identify residents who would benefit from follow-up care and coordinate with their primary care provider.
  • Document every encounter accurately and completely, to Medicare standards, in our EHR.
  • Represent Total Life professionally with residents, families and community staff.
  • Travel to assigned senior living communities in the Capital Region (Albany-Schenectady-Troy) area.
  • Lead educational talks for residents on healthy aging.

Requirements

  • Active, unrestricted New York RN license and Nurse Practitioner certification from the New York State Education Department, in a specialty that covers older adults.
  • Current national NP board certification.
  • Active DEA registration, New York electronic prescribing (EPCS) setup, and completion of New York's mandatory prescriber education.
  • More than 3,600 hours of NP practice, or the ability to practice under a written collaborative agreement and practice protocols with a physician, as required in New York.
  • Clinical experience caring for adults 65+.
  • Experience with Medicare documentation and E/M coding.
  • Own malpractice insurance (minimum $1M per occurrence, $3M aggregate) that covers in-person care, or the ability to obtain it before your first site day.
  • Comfort presenting to groups and speaking with older adults and their families.
  • A valid driver's license and reliable transportation.
  • Ability to complete each community's onboarding requirements, which may include a background check, TB screening and immunizations.
  • Weekday availability for on-site days in the Capital Region (Albany-Schenectady-Troy) area.
