About the Role
The Community Care & Outreach Supervisor is responsible for providing services to the community in their assigned program, including information, crisis intervention, developing service plans, and facilitating referrals. This role also involves recruiting, training, evaluating, and supervising Care Management and outreach staff.
Responsibilities
- Performing duties as discussed in Care Manager and Street Outreach job duties.
- Recruiting, training, supervising and evaluating assigned Care Management and Street Outreach staff.
- Coordinating the referral process including screening in accordance with program procedures.
- Reviewing service delivery to monitor case progress and ensure effectiveness of service plans.
- Serving as liaison with community agencies and referral sources for service coordination, referral, outreach, and resident training.
- Coordinating appointments for participants with community service providers for employment training, education, mental health, substance abuse, and other needs.
- Providing assistance in crisis intervention and stabilization.
- Compiling and maintaining statistical and other reports per requirements of funding agencies and Hudson River Housing.
- Coordinating and providing oversight to in-house programming, including facilitating on-site psychosocial, educational, and recovery groups.
- Coordinating and providing oversight to all canvassing, presentation, and community events the outreach team participates in.
- Overseeing participants’ compliance with site policies and procedures.
- Carrying out additional tasks as required by supervisory or HRH Administrative staff.
Requirements
- BA/BS in human service field or experience in human service capacity.
- Equivalent experience and training will be considered in lieu of degree.
- Experience providing case management.
- Valid driver’s license.
- Personal telephone required.
- Good working knowledge of local human service network.
- Good motivational techniques.

- Strong communication skills.
- Sensitivity toward the program’s target population.
