About the Role
Stern at Home Therapy is expanding and hiring Physical Therapists (PTs) to support growth in the Buffalo, NY area. This role offers opportunities to treat a primarily geriatric patient population in their homes, with clinical autonomy to develop individualized plans of care, supported by an administrative team.
Responsibilities
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and the interdisciplinary care team.
- Coordinate with our administrative team to develop a caseload within your preferred Buffalo-area territory.
Requirements
- Active Physical Therapist license in New York required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently in a home-based setting.
- Home health or geriatric experience is preferred but not required.

Salary/Compensations
- Full-Time W-2 Tiered Per-Visit Option: $45–$60 per follow-up visit, $65 per 4-unit evaluation. Rate increases with patient volume.
- Full-Time 1099 Tiered Per-Visit Option: $60–$70 per follow-up visit, $75 per evaluation. Rate increases with patient volume.
- Part-Time W-2: $55 per follow-up visit, $65 per 4-unit evaluation.
- Part-Time 1099: $62 per follow-up visit, $75 per evaluation.
Benefits
- Full-Time W-2: 401(k) with annual match (1.5% to 7%), Paid Time Off (2 weeks after 6 months, 3 weeks after 9 months), 5 paid holidays, student loan repayment assistance ($125/month after 1 year), $500 annual CEU stipend, $250 commuter stipend, childcare assistance program in development, equipment provided.
- Part-Time W-2: 401(k) with annual match, student loan repayment assistance, CEU support, prorated commuter stipend (up to $175/month), equipment provided.
