About the Role
Delivers pharmaceutical care through patient-centered clinical services, medication information, education, and preparation/distribution to ensure safe, effective, and cost-efficient medication therapy. Exercises sound judgment and clinical skills to contribute to patient care plans based on diagnosis. Builds confidence and cooperation through competent patient assessment, monitoring, and effective communication. Adheres to all regulations, codes, policies, and procedures for privacy and safety.
Responsibilities
- Provide patient-centered clinical services.
- Provide medication information and education.
- Prepare and distribute medications.
- Ensure safe, effective, and cost-efficient medication therapy.
- Provide input to and implement patient care plans.
- Perform patient assessments.
- Monitor patient care.
- Communicate effectively with patients, families, and healthcare providers.
- Adhere to all local, state, and federal regulations, codes, policies, and procedures.
- Ensure patient privacy and safety.
- May be responsible for performing specific tasks.
- May orient other staff to the department.
- Manage, store, prepare, receive, unpack, transport, dispose of, or administer hazardous drugs in accordance with USP 800 guidelines.
Requirements
- Graduate of an accredited pharmacy school.
- Registered Pharmacist within 120 days.
- 1 year of recent relevant experience.
- Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations, and laws and regulations.
- Knowledge and understanding of different Pharmacy practice settings, including narcotic delivery and procedures.
- Knowledge and ability to identify and employ pharmaceutical and medical terms, abbreviations, and symbols commonly used in prescribing, dispensing, and record keeping of medications.
- Basic working knowledge of legal requirements and accreditation standards including National Association of Boards of Pharmacy (NABP), The Joint Commission (TJC), Title XXII, United States Department of Homeland Security (DHS), Drug Enforcement Administration (DEA), Food and Drug Administration (FDA) and United States Pharmacopeia (USP).
- Written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences.
- Well-developed time management and organizational skills.
- Ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadlines.
- General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), related pharmaceutical technology, EHR, and EPIC.
- Ability to work independently and as part of a team.
- Ability to accomplish multiple tasks in an environment with interruptions.
- Ability to identify, evaluate, and resolve standard problems by selecting appropriate solutions from established options.
- Ensure the privacy of each patient’s PHI.
