About the Role

The Community Health Worker provides care coordination and insurance navigation as part of a collaborative team. They work directly with patients, many of whom are high risk, along with their families and primary care providers, to ensure smooth and continuous care. The role involves patient outreach, providing transportation and enabling services, and helping close gaps in care. The CHW also assists patients in obtaining and maintaining health insurance, including applications and renewals for Medicare, Medicaid, Essential Plan, and Marketplace plans, explaining work requirements, and ensuring timely paperwork submission. They act as a point of contact, advocate, and information resource for patients, care teams, families, caregivers, payers, and community resources. The CHW must complete the Facilitated Enroller Certification Training Program by June 30, 2027.

Responsibilities

  • Provide care coordination and insurance navigation.

Requirements

  • Certification as a Community Health Worker or completion of a Community Health Worker training program.
  • At least one year of experience as a Community Health Worker or Case Manager in a Community-Based Organization.
  • Current New York State LPN license.
  • Certification as a Medical Assistant.
  • Certification as a Nursing Assistant with proven knowledge of patient care, barriers of care faced by high risk populations, and care coordination of high risk population.
  • A degree in the Human Services field.
  • Willingness to learn administrative skills including knowledge of patient care, medical terminology, access, and compliance issues.
  • Valid driver's license and means of personal transportation.
