About the Role
Provide individual/family counseling, treatment planning, assessments, and possible crisis intervention virtually or in-person for children, adolescents and their families to help prevent the progression of behavioral health needs in a community setting.
Responsibilities
- Provide all clinical services within the context of the child and family environment, primarily in the child's home, school, or preferred community location.
- Offer telehealth services as appropriate and in alignment with state regulations and program procedures.
- Provide crisis management by stabilizing the child/youth in the home after the crisis has occurred and/or assist with goal setting.
- Provide clients and their families with goal-directed community services and solution-focused interventions intended to address challenges associated with a behavioral health need and to achieve identified goals or objectives as set forth in the client’s treatment plan.
- Collaborate with the child and family to develop and establish a diagnosis and treatment plan that meets medical necessity while honoring the child’s self-identified needs, goals, and ethnic, religious, and cultural identities.
- Establish a diagnosis using the Diagnostic Statistical Manual-V (DSM V) and relevant assessment tools to establish a medically necessary treatment plan that demonstrates measurable goals, objectives and supports.
- Work collaboratively with the child and family system of care, which could include Residential, Behavioral Health Clinic, Foster Care, Preventative services, Health Homes, Care Coordination, school districts, and primary care physicians.
- Complete progress notes via Electronic Clinical Record (ECR) within 24 hours of service provision.
- Understand and adhere to all agency policies and procedures, including meeting Productivity Expectations.
- Understand and adhere to Gateway's Code of Conduct, Code of Ethics, providing professional and meaningful interventions and interactions.
- Participate in clinical supervision, case consultation, staff meetings, and trainings as required by the program.
- Maintain a minimum of 10 billable client service hours per week.
- Service clients in Erie and Niagara Counties, with reasonable discretion in case acceptance by geography.
- Attend required supervision and case consultation; attendance at staff meetings is preferred.
- Stay informed of department updates, agency policies, and procedural changes, even when unable to attend meetings.
- Monitor agency email and phone regularly, with a response time of no more than 24 hours on business days.
- Complete all clinical documentation within established agency timeliness and maintain compliance with regulatory and professional standards.
Requirements
- Excellent documentation skills
- Ability to work independently and also be an enthusiastic team player
- Ability to work nights and/or provide non-traditional hours, at least 2 days per week.
- Ability to work effectively with clients, staff, families, and community contacts
- Ability to travel to community locations to meet with clients
- Valid New York State driver’s license, adequate auto insurance, and reliable transportation to travel locally on a regular basis is required.

Education Level
- Must be license-eligible, hold a provisional license (LMHC-P or LMSW permit), or be fully licensed as an LMSW, LCSW, or LMHC.
- LMSW candidates should be eligible for or actively pursuing their “C” designation.
- LMHC candidates should be eligible for or actively pursuing their Diagnostic Privilege/Permit.
- Candidates may also qualify under grandfathering provisions, with appropriate exemption documentation provided.
Salary/Compensations
- $50,000 - $58,850
