About the Role
BrightStar Care of Texas is seeking a compassionate and skilled Physical Therapist to join their home health team, focusing on providing meaningful care to patients in their homes.
Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
- Deliver skilled PT services to home health patients across a variety of diagnoses and conditions.
- Monitor patient progress and adjust care plans accordingly.
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated, patient-centered care.
- Maintain accurate, timely clinical documentation in compliance with state and federal regulations.
- Educate patients and family members on home exercise programs and safe mobility techniques.
Requirements
- Active Texas Physical Therapist (PT) license required.
- Home health experience preferred; new graduates with clinical experience are welcome to apply.
- Strong communication, time management, and organizational skills.
- Reliable transportation and valid driver's license.
- CPR certification preferred.
