About the Role

The Behavioural Support Personal Support Worker provides direct care, education, and support to residents demonstrating behaviors associated with dementia, delirium, mental health, addictions, or other neurological conditions. This role contributes expertise in personal care and activities of daily living for residents facing responsive expression challenges, coordinating with stakeholders for a person-centered approach to resident care.

Responsibilities

  • Contribute to delivering person-centered care by promoting resident abilities and self-esteem.
  • Develop strategies to promote optimal quality of life for residents with responsive behaviors.
  • Conduct assessments using various tools to describe resident behaviors, identify risks, and determine caregiver strategies.
  • Ensure the plan of care is regulated with responsive behaviors, including weekly rounds to mitigate risks.
  • Communicate with external stakeholders and management to identify risks associated with responsive behavior.
  • Demonstrate participation in quality improvement activities and provide monthly reports.
  • Provide coaching and mentorship to staff to ensure collaboration of care for responsive behaviors.
  • Conduct resident care reviews and attend quality program meetings.
  • Collaborate with pharmacists and physicians for consultation regarding medication effects on residents.
  • Participate in staff development programs and attend ongoing training to support skills in managing residents with responsive behaviors.
  • Assist in tasks considering home policies and the Behavioral Support Ontario project.
  • Maintain positive relationships with all stakeholders, including staff, residents, and the public.
  • Provide excellent customer service and assist with administrative duties.
  • Perform other duties as required.

Requirements

  • Completed a PSW Program meeting appropriate standards (Ministry of Training, Colleges and Universities, National Association of Career Colleges, or Ontario Community Support Association) with a minimum of 600 hours duration (including class and practical experience).
  • Knowledge and proven experience working with residents in Long-Term Care.
  • Knowledge of GPA, P.I.E.C.E.S., U-First, or willingness to receive such training.
  • First Aid and CPR certification.
  • A serving heart and a passion for caring for seniors.
  • Ability to interact with residents, families, and staff in a positive and supportive manner.
  • Ability to work individually and as part of a team, prioritizing tasks by urgency.
  • Excellent communication (verbal and written) and comprehension skills.
  • Promote safe work practices and ensure compliance with the Occupational Health and Safety Act.
  • A negative vulnerable sector check completed within 6 months of the start date.
  • TB screening documentation as required under the Southbridge Care Homes’ Tuberculosis (TB) Screening Policy.
