About the Role
The City's Seniors Services and Long-Term Care Division supports Toronto seniors and residents in long-term care homes through exceptional care and services. Responsibilities
- Assess residents’ rehabilitative needs through observation.
- Participate in multidisciplinary team meetings to develop and modify resident care plans, providing input on requirements and progress.
- Develop and implement rehabilitation programs to improve or maintain residents' level of functioning.
- Develop and document plans for therapeutic group programs, including resources, goals, content, scheduling, and evaluation methods.
- Conduct treatment and instruction programs for ambulation, mobility, strengthening, positioning, sensory stimulation, reality orientation, and activities of daily living.
- Utilize applicable equipment and devices for rehabilitation.
- Set up treatment areas.
- Monitor residents’ progress and adjust treatment as appropriate.
- Evaluate rehabilitation programs on a regular basis.
- Recommend and implement modifications to treatment techniques.
- Record observations and progress on resident charts.
- Document assessments, program plans, and ongoing evaluations using Point Click Care (PCC) software.
- Prepare quarterly summaries.
- Request orders for equipment and supplies for residents.
Requirements
- Completed a post-secondary program in restorative care or have the combined equivalent of education and experience.
- Considerable experience in a Long-Term Care, Geriatric or Chronic Care setting providing individual and group rehabilitation programs.
- Experience in conducting functional resident assessments and evaluating therapeutic programs.
- Fundamental knowledge of physiology, disease process, and disabilities related to elderly persons.
- Fundamental knowledge of physiology, disease process, and disabilities related to young adults requiring long-term care.
- Fundamental knowledge of adaptive and assistive devices.
- Verbal communication skills to interact with staff and residents.
- Written communication skills to complete documentation.
- Ability to work cooperatively with staff members.
- Ability to develop and provide programs and services to residents.
- Ability to deliver a high level of Customer Service, Fiscal Accountability, Innovation, Result Orientation, and Teamwork.
- Ability to motivate residents to participate in programs.
- Sensitivity and understanding of residents experiencing change.
- Flexibility to adapt to changing resident conditions.
- Ability to work a variety of shifts, including evenings and weekends.
- Ability to support Toronto Public Service values to ensure a culture that champions equity, diversity, and respectful workplaces.
- Selected candidates will be required to provide a satisfactory Police Reference Check (PRC) with vulnerable sector screening, tuberculosis screening, and proof of immunization.
