About the Role
The Habilitation Support Professional (HSP) supports individuals with Intellectual and Developmental Disabilities in Home and Community Based Settings (HCBS) and Day Habilitation programs. The HSP follows the Individualized Service Plan (ISP) and/or Day Habilitation Plan to promote independence and person-centered decision-making.
Responsibilities
- Ensures client safety in home or day habilitation settings, promoting independence and meeting client needs and choices in a person-centered manner.
- Attends to the personal hygiene needs of clients.
- Collaborates with clients, guardians/advocates, and supervisors to identify valued outcomes for the habilitation plan.
- Implements valued outcomes daily using agency documentation procedures.
- Develops and implements activities with clients and their primary caregivers.
- Accompanies clients on trips and outings, supervising activities.
- Participates in interdisciplinary team treatment planning meetings to review progress and plan future outcomes.
- Shares and documents client experiences and observations with the treatment team through supervision and the electronic health record.
- Contacts supervisor for any medical or behavioral occurrences outside the client's baseline.
- Understands and follows all safeguards listed in individual treatment plans.
- May dispense medications in Day Habilitation settings if needed.
- Completes OPWDD documentation requirements, including monthly checklists and summaries.
- Follows agency guidelines for timekeeping and mandatory training.
- Completes ongoing OPWDD and other required trainings.
Requirements
- High School diploma or a minimum of 1 year of experience working with the IDD population.
- Must be at least 18 years of age.
- AMAM certification required for the Day Habilitation setting during the probationary period.
- Basic computer skills required, including agency software programs and Electronic Health Record.
- Ability to review treatment plans and document daily logs.
- Dexterity to pick up small items for client activities.
- Ability to lift clients using a mechanical device, position clients, push clients in wheelchairs, and physically transfer clients where prescribed.
