About the Role

Health Center Inc. is seeking a Patient Care Clerk for their Patient Services Department to manage the off-site clinical care referral process. This role serves as a liaison between the Health Centers and off-site providers, ensuring timely and authorized referrals while maintaining patient confidentiality.

Responsibilities

  • Coordinate all approved off-site appointments with participating providers using knowledge of organizational guidelines, eligibility rules, and covered benefits.
  • Serve as the liaison between the Health Centers and off-site providers involved in patient care.
  • Ensure referrals are authorized and scheduled within an appropriate timeframe.
  • Follow the Patient Services workflow process from implementation to timely completion.
  • Maintain up-to-date knowledge of all external partners’ requirements and guidelines related to off-site referrals and communication methods.
  • Follow organizational policies and procedures for the provision of off-site referrals.
  • Serve as a resource for providers and patients for the explanation of the off-site referral process.
  • Appropriately escalate external resource issues and opportunities for improving referral care delivery to the supervisor.
  • Ensure patient confidentiality and follow all HIPAA and governmental/state agency requirements regarding PHI.
  • Perform additional job-related clerical duties such as answering phones, copying, data entry, and scanning as needed.
  • Represent the Health Center to outside providers in a professional manner.
  • Update patient demographics and other data on EMR as needed.
  • Provide excellent customer service.
  • Contribute to team effort.
  • Assist in other areas as needed.
  • Perform other duties as assigned.

Requirements

  • 2+ years of related work experience in a healthcare setting.
  • Proficiency with Electronic Medical Record (Cerner preferred).
  • Ability to manage and effectively follow up on completion of assigned duties.
  • Demonstrated ability to take ownership of assigned duties and responsibilities.
  • Strong ability to maintain positive and professional relationships with peers.
  • Positive work attitude.
