About the Role

The Per Diem Primary Care Provider (PDPCP) acts as a clinical leader, providing quality primary care services to Lyon-Martin patients, contributing to practice growth and promoting team functioning. The PDPCP collaborates with a supervising physician and upholds a commitment to addressing systemic discrimination within the organization and clinical practice.

Responsibilities

  • Provide comprehensive care within the scope of credentialed privileges.
  • Document care promptly, closing visit notes and responding to in-basket messages within 72 hours.
  • Analyze and interpret patient histories, symptoms, findings, and diagnostic information to develop diagnoses.
  • Ensure appropriate follow-ups for all ordered care, including referrals and test results.
  • Consult with the Medical Director and other physicians for treatment options as needed.
  • Maintain familiarity with and utilize standardized procedures to guide care.
  • Complete patient sessions in a timely manner.
  • Provide clinical leadership to support staff.
  • Collaborate with mental health staff and specialists to improve patient outcomes.
  • Meet benchmarks for quality indicators.
  • Assist in continuous quality improvement through needs assessment and problem-solving.
  • Maintain availability for administrative and clinical duties, which may include case presentations, staff training, supervision, and committee leadership.
  • Attend continuing professional education programs as required.
  • Represent Lyon-Martin at professional conferences, trainings, and meetings.
  • Obtain and maintain licensure, DEA registration, and BCLS/CPR certification.
  • Complete all mandatory trainings annually.
  • Comply with Lyon-Martin Policies and Procedures, agreements with insurers, and all applicable statutes and regulations.
  • Perform other duties as assigned.

Requirements

  • At least 1 year of experience as a licensed, practicing clinician.
  • Graduation from an accredited program for Nurse Practitioners, Physician Assistants, Doctor of Medicine (MD), or Doctor of Osteopathic Medicine.
  • Valid California license as an AGNP or FNP with Furnishing license OR current, valid California License to practice as a Physician Assistant OR valid California License to practice as a Physician and Surgeon or Osteopathic Physician and Surgeon.
  • Possession of DEA Controlled Substances Registration Certificate.
  • Possession of NPI.
  • Current BLS/ACLS certificate.
  • Experience working with lesbian, bisexual, transgender, gender non-conforming, non-binary, and intersex communities.
  • Experience working with diverse populations including those with disabilities, varying racial, ethnic, cultural backgrounds, and economic statuses.
  • Experience working with specific populations such as sex workers, those experiencing homelessness, individuals with mental illness or substance use challenges, those with a history of incarceration, and people of varying ages.
  • An interest in and commitment to gender-affirming health care and health care access for underserved populations.

  • $111.50 per hour (MD or DO)
  • $76.14 per hour (NP or PA)
