About the Role
This role provides high-quality infusion care directly in patients' homes, ensuring a patient-first experience. You will support physicians, caregivers, and internal teams through clear communication and dependable clinical coordination, helping patients feel safe and supported while managing complex therapies.
Responsibilities
- Provide direct home infusion nursing care including IV insertion, monitoring, and medication administration
- Monitor patients for adverse reactions, obtain vital signs, and complete physical assessments
- Perform venipuncture and manage venous access including peripheral IVs and central lines
- Educate patients and caregivers on home infusion therapy safety
- Collaborate with physicians, nursing coordinators, and internal care teams
- Coordinate start-of-care and ongoing visits
- Document nursing notes, therapy administration reports, and care plans
- Communicate patient status changes to referring providers
- Maintain compliance with federal and state regulations and privacy requirements
- Participate in staffing coordination and scheduling
- Travel within the state to patient residences and care locations
Requirements
- Current RN license in New York
- Minimum of two years of RN experience
- IV certification
- Experience with central line care and management
- Willingness to travel locally to patient residences and healthcare facilities
- Participation in an on-call rotation approximately once every eight weeks
- Ability to lift and/or move up to 25 pounds
