About the Role

The Clinical Dietitian is an integral member of the interdisciplinary health professional team, responsible for patient nutrition care, disease management, and education. This role involves initiating cost-effective nutrition therapy through patient assessment, developing and implementing patient-centered care plans, and ongoing evaluation. The dietitian focuses on outcome-oriented, evidence-based nutrition care to optimize clinical and fiscal outcomes, emphasizing health promotion and disease prevention. Responsibilities include managing the continuum of patient care with effective strategies for self-management of disease to decrease complications and hospital admissions, and educating patients/caregivers on disease and nutritional management. The dietitian promotes a healthcare team approach and provides health assessment, interventions, and follow-up in accordance with relevant acts, regulations, policies, standards, and guidelines.

Responsibilities

  • Provide nutrition care, disease management, and education to patients.
  • Initiate cost-effective nutrition therapy including patient assessment, care plan development, implementation, and evaluation.
  • Develop outcome-focused nutrition care plans incorporating evidence-based best practices.
  • Optimize clinical and fiscal outcomes through nutrition therapy.
  • Emphasize health promotion and disease prevention.
  • Manage the continuum of patient care with effective strategies for self-management of disease.
  • Educate patients and caregivers about disease and nutritional management.
  • Promote a healthcare team approach to patient care.
  • Provide health assessment, interventions, and appropriate follow-up.

Requirements

  • Bachelor of Science in Human Nutrition at a recognized University required.
  • Completion of Dietetic internship (a 1 year integrated or post graduate program) required.
  • Licensure by Nova Scotia Regulator of Dietetics required.
  • Current Membership in Dietitians of Canada preferred.
  • Two years of related experience in an adult health care facility preferred.
  • Specialty Practice Certification such as Certified Diabetes Educator (CDE) considered an asset.
  • Access to reliable transportation for travel for work.
  • Competencies in other languages an asset; French preferred.
