About the Role
Link Rehab and Wellness is seeking a Licensed Clinical Social Worker to provide clinical assessments, therapy, and counseling services to geriatric individuals with mental health challenges. The role involves developing treatment plans, delivering interventions, and collaborating with a multidisciplinary team to enhance resident well-being.
Responsibilities
- Conduct comprehensive behavioral health assessments, including intake interviews, screenings, and evaluations.
- Develop, implement, and revise individualized treatment plans based on client goals, preferences, and clinical needs.
- Provide evidence-based therapeutic interventions, including individual, group, and family therapy.
- Coordinate and collaborate with other healthcare professionals and community organizations for comprehensive care.
- Provide crisis intervention services, including de-escalation and safety planning.
- Maintain accurate and timely clinical documentation in accordance with ethical, legal, and organizational standards.
- Educate clients and families about mental health, coping strategies, and available resources.
Requirements
- Licensed Clinical Social Worker (LCSW) required.
- Licensed in the state of New Jersey.
