About the Role

The Day Center Aide is an integral member of the PACE at Hudson Headwaters (PHH) team, responsible for providing support and assistance to Participants during appointments and Day Center activities. This role collaborates with the PACE Interdisciplinary Team to ensure Participants’ care needs are met and their care plan is followed.

Responsibilities

  • Escort Participants to outside appointments and provide assistance when necessary.
  • Assist drivers with loading and unloading Participants from vehicles.
  • Assist the transportation department with Participants’ take-home supplies.
  • Provide and document care in accordance with established policies, guidelines, and applicable regulations.
  • Adhere to PHH safety requirements and Participant care plans regarding use of assistive devices.
  • Assist Participant Care Associates and other clinical staff with transfers and services including grooming and oral care.
  • Provide supervision and support to Participants, monitoring and responding to behavioral disturbances.
  • Respond appropriately and in a timely manner to personal alarm devices.
  • Assist Participants with restorative and supportive care such as ambulating, positioning, transferring, filling oxygen, and providing other support.
  • Follow clinical instructions as assigned.
  • Maintain accurate and current Participant care and service logs.
  • Assist the dietary staff with meal service and clean-up of dining areas.
  • Maintain personal care areas including coatroom, bath/shower rooms, personal care/linen supply areas, Participant rooms, and laundry areas.
  • Assist with stocking and maintaining personal care supplies for use in the Day Center.
  • Assist with laundry duties.
  • Assist with implementing recreation programs in a group setting and 1:1.
  • Work collaboratively with other departments to ensure Participant needs are met and a clean, sanitary, and safe environment is maintained.
  • Understand and adhere to PHH policies and procedures and PHH's Participant Rights Policy.
  • Protect privacy and maintain confidentiality of sensitive Participant information.
  • Attend staff and Participant care planning meetings as requested, and department in-services as required.
  • Report any event, accident, or unusual occurrence to the appropriate personnel.
  • Additional work functions and duties may be assigned.

Requirements

  • Requires reliable transportation.
  • Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
  • Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.
  • Have not been excluded from participating in Medicare and Medicaid programs.
  • Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.
  • Ability to work within a care team with initiative, imagination, resourcefulness, and flexibility.

Skills

  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards
  • Requires the exertion of 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects
  • May also need to move heavy equipment, transfer/lift patients and assist patient with walking and turning
  • Stoop, kneel, crouch, lift, and stand for long periods

Work Type

  • 40 hours per week
  • Monday-Friday, 8am-4:30pm

Experience Level

  • 1 year of experience working with a frail or elderly population or be willing to receive appropriate training upon hire

Education Level

  • High School Diploma or GED
  • A current and valid state-issued Personal Care Assistant (PCA) certificate is required
  • Current BLS certification or willing to complete certification
  • Meet a standardized set of competencies for the specific position description established by PACE before working independently

Salary/Compensations

  • $21.50/hour
