About the Role

The Cardiology Structural Heart Clinical Manager will support the Director of Cardiology Acute Care and Ambulatory Advanced Practice Providers in ensuring successful operations, fiscal management, and quality of the structural heart program. This leader is responsible for overseeing day-to-day clinical and operational functions while providing effective people management and employee relations support to the care team. The incumbent will foster a high-performing, engaged workforce by promoting accountability, professional development, and a collaborative team environment. The leader will also demonstrate and champion the organizations mission, vision, values, and service excellence standards, helping to build and sustain a culture focused on exceptional outcomes in patient safety, clinical quality, patient experience, and team member engagement and satisfaction. Note: Role is 75% clinical/25% administrative/leadership.

Responsibilities

  • Facilitate team coordination of duties with regular review of processes, priorities, and efficiency.
  • Arrange opportunities for outreach and public education in collaboration with the program director.
  • Coordinate work schedules for Structural Heart APP and RN clinicians.
  • Manage PTO requests and coverage for structural heart service APPs and RNs.
  • Track time and attendance, and approve payroll.
  • Assist with day-to-day operations such as IT access and issues, and sick call coverage.
  • Participate in the interview process of new candidates.
  • Help to coordinate precepting and training of new clinical team members and students.
  • Assist with execution of operational objectives and strategic priorities in the creation and achievement of goals.
  • Provide active people management and employee relations support by coaching, mentoring, and developing team members to achieve optimal performance.
  • Consistently monitor performance, provide real-time feedback, and ensure understanding of expectations.
  • Lead through influence while fostering a collaborative and respectful team environment.
  • Address employee relations matters proactively, including performance concerns, interpersonal conflicts, and behavioral issues.
  • Partner with HR and leadership to ensure fair, consistent, and compliant resolution of employee relations matters.
  • Help to develop, monitor, and modify protocols, procedures, and processes to streamline patient care and improve workflows.
  • Work with clinical leadership to adopt and implement quality initiatives and outcome reporting processes specific to the structural heart program.
  • Effectively communicate with colleagues across the system.
  • Participate in hospital, ambulatory, HVI, and/or network-focused operational and clinical committees as relevant and appropriate.
  • Identify issues, perform gap analysis, prioritize business needs, and execute solutions.
  • Escalate issues and concerns proactively to the Director as needed.
  • Maintain and model Northwell Health Values.
  • Perform other duties as required.
  • Coordinate all services and systems required for an organized multidisciplinary team approach to the evaluation, treatment, and follow-up of patients requiring structural heart procedures.
  • Coordinate material, plan agenda, and attend weekly multidisciplinary structural heart program meetings.
  • Attend weekly valve clinic with physicians, including patient assessment for procedure qualification.
  • Perform prn and follow-up office visits for structural patients before and after their procedure.
  • Receive incoming referrals from providers and respond in a timely manner.
  • Triage referrals according to criteria to ensure patient needs are met on a priority level.
  • Order and manage clinical tests according to protocol to complete the assessment process.
  • Initiate and provide outside referrals to specialists for patients as needed.
  • Coordinate structural procedure schedule and communicate patient flow with providers and staff.
  • Educate patient and family about the referral, screening process, evaluation steps, procedure scheduling, and follow-up.
  • Communicate to referring providers the plans for patient evaluation and procedures.
  • Serve as the content expert for structural procedures and assist other staff.
  • Work closely with vendor clinical specialists for updates to device changes and training for internal staff.
  • Provide case planning for individual patient care.
  • Develop and revise policies and procedures with the program director surrounding structural patient care.
  • Implement and educate internal staff on new policies and procedures.
  • Participate in TVT Registry for collection of data and quality improvement.
  • Interface with the Research Department for patients who may be candidates for participation in structural heart related trials.
  • Work with IT to develop/revise electronic medical record orders for evaluation and clinical care.
  • Provide information on hospital and professional billing to applicable personnel to ensure billing compliance.

Requirements

  • Master degree or higher in nursing or physician assistant studies
  • Minimum of three (3) years of cardiology APP experience, or equivalent experience
  • Lead APP or supervisory and/or project management experience
  • New York State Nurse Practitioner License or Physician Assistant License
  • National Board Certification as a Nurse Practitioner or Physician Assistant
  • Advanced Cardiac Life Support (ACLS)
