About the Role

Responsibilities

  • Conduct and coordinate resident assessments according to State, Federal, and Company guidelines.
  • Oversee the generation and electronic transmission of Minimum Data Set (MDS) for Medicare patients within mandated timeframes.
  • Coordinate the development of resident assessments (MDS), including interviews, evaluation of resident condition, and timely transmission.
  • Develop and implement procedures for newly admitted residents with the Director, Clinical Services.
  • Assist in scheduling resident assessment/care plan meetings and coordinate participation of outside care team members.
  • Coordinate the development of a written plan of care identifying resident needs, goals, and responsible professional services.
  • Ensure care plans include measurable objectives and timetables.
  • Maintain and update policies and procedures for resident assessment and care plan development.
  • Ensure availability of the MDS Instructor’s Manual.
  • Develop and implement a quality assurance program for resident assessment/care plans.
  • Monitor QI and QM reports for corrective action implementation.
  • Assist with the care plan portion of resident discharge plans.
  • Participate in community surveys conducted by government agencies.
  • Provide leadership and participate in committees such as Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
  • Develop and participate in in-service training classes on assessment skills and MDS functions.
  • Assist in developing training activities related to resident assessment/care plan skills.
  • Attend and participate in annual community in-service training programs.

Requirements

  • Must possess a Nursing Degree from an accredited college or university.
  • Minimum of two (2) years of experience as a supervisor in a hospital, nursing care community, or other related health care facility.
  • Minimum of six (6) months training experience in rehabilitative and restorative nursing practices.
  • Knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, regulations and guidelines governing long-term care.
  • Current State RN license.
  • Uses limited independent judgment to make decisions based on precedents and established guidelines.
  • Solves problems using standard procedures and precedents.
  • Knows when to refer issues to supervisor and when to handle them personally.
  • Has a working knowledge of a skill or discipline that requires basic analytic ability.
  • Has an overall understanding of the work environment and process.
  • Has working knowledge of the organization.
