About the Role
The Behavioural Interventionist will support staff and informal caregivers in providing care to individuals with unmanaged responsive behaviours that impede care delivery. These behaviours may be associated with Alzheimer’s disease and related dementias, mental health, substance use, and other neurological conditions.
Responsibilities
- Conduct behavioural assessments as per Alzheimer Society Peel best practice guidelines.
- Collaborate with patients, caregivers, and hospital staff to identify behavioural triggers.
- Implement, evaluate, and document specific behavioural interventions in consultation with an elder life specialist.
- Develop, monitor, and evaluate care plan recommendations by reviewing documentation, behavioural tracking, and outcome measures.
- Provide patient-specific education, training, modeling, and coaching to hospital staff.
- Create appropriate referrals to other BSO team members to support patients, caregivers, and staff.
- Support successful patient transitions from hospital to the next location through collaboration with community services and pre-transition planning.
- Develop individualized transitional care plans shared with families and caregivers.
- Provide necessary information to patients and caregivers to support transitions.
- Ensure patients and caregivers receive timely information and referrals to the Alzheimer Society Peel and BSO services.
- Identify staff and caregivers who would benefit from additional education and make appropriate referrals.
- Encourage independence.
- Implement protocols to assist in managing behaviours and facilitating care.
- Coach staff in understanding and managing responsive behaviours.
- Conduct direct observation of patients during responsive behaviours to identify triggers/unmet needs and recommend management strategies.
- Participate in care conferences/patient huddles as required.
- Maintain ongoing contact with medical health care professionals.
- Initiate and participate in ongoing collaboration with community partners.
- Maintain accurate and timely monthly statistics.
- Prepare and submit required administrative documentation.
- Participate in administrative team meetings, providing input for service improvements and quality initiatives.
Requirements
- Community college diploma in a health care related field (Bachelor’s degree preferred).
- Certificate in gerontology preferred or demonstrated evidence towards completion.
- Gently persuasive approach certified.
- One year recent experience working with seniors and individuals with cognitive impairment in an acute care setting.
- Experience with the prevention and management of geriatric syndromes such as delirium/BPSD preferred.
- Attention to detail and expectation to meet documentation standards.
- Excellent communication (written and verbal) and interpersonal skills.
- Demonstrated ability to work as part of an inter-professional team.
- Ability to work independently: initiating, prioritizing, and managing multiple tasks.
- Must be willing to travel between all Osler sites.
