About the Role

Provide professional patient care to long-term and post-acute care residents. Direct and monitor the care provided by CNAs, ensuring proper documentation and recording. Lead CNAs and ensure quality patient care.

Responsibilities

Administer medications accurately and in a timely manner for the assigned team, along with concurrent required documentation.
Perform treatments and document progress as per physician orders and facility protocol.
Ensure concise, pertinent, and complete resident care documentation using the computerized medical record process.
Initiate appropriate care pathway tools.
Direct CNAs in the timely completion of quality individualized personal care in compliance with resident care plans.
Ensure CNA documentation is accurate and complete.
Receive and give relevant reports, both written and verbal, from/to appropriate staff as per departmental protocols.
Contact physicians, transcribe orders, and follow up to institute orders in a timely manner.
Ensure proper monitoring of residents and relevant condition status are reported to the charge nurse/supervisor and/or physicians promptly.
Complete pre-reviews of medication administration records as per facility protocols.
Complete required MDS documentation and all related observations in a timely manner.
Support the CNA team by assisting in direct ADL care of residents.

Requirements

Graduate from an accredited School of Practical Nursing.
Current LPN licensure in the State of New York.
Basic Life Support (BLS) Certification from the American Heart Association (AHA).
Must complete 40 hours per month, including 16 weekend hours.
Moderate clinical role requires continual movement, including standing, walking, and lifting 20–50 lbs.
Pushing and pulling medical equipment and assisting with patient positioning.
Frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments.
High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns.
Substantial emotional and cognitive demands, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus.
Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation.
TB testing, mask-fit testing, and physical exams are common requirements.
