About the Role
The Montefiore Licensed Practice Nurse contributes to the standards of nursing practice, actualizes the vision, mission, and values of Montefiore Medical Center, and participates in performance improvement and research activities. The LPN maintains professional practice standards, demonstrates leadership skills, and provides direct patient care within their scope of practice.
Responsibilities
- Provide patient/family teaching and discharge instructions.
- Initiate measures for patient support.
- Identify and report patient needs.
- Implement teaching methodologies for identified learning needs.
- Facilitate discharge in collaboration with RN.
- Set priorities, problem-solve, and use proper judgment in difficult situations.
- Adapt to change in the work environment and manage competing demands.
- Administer medications with 100% compliance.
- Maintain a safe environment for patients/families.
- Document direct patient care accurately and timely.
- Participate in continuous quality improvement initiatives.
- Communicate effectively with patients/families and healthcare team members.
- Incorporate principles of family-centered care.
- Ensure patient rights are respected.
- Demonstrate respect for cultural diversity.
- Adhere to established therapeutic relationships with patients/families.
- Collaborate with registered nurse and co-workers to resolve patient care problems.
- Utilize communication systems to problem-solve.
- Report on and off duty to the designated RN.
- Ensure call bells are answered in a timely manner.
- Speak clearly and persuasively.
- Listen and seek clarification.
- Be responsible and accountable for own practice.
- Adhere to MMC departmental and Human Resources policies and procedures.
- Support the mission, vision, and goals of MMC.
- Complete all mandatory LPN nursing competencies.
- Attend educational programs as scheduled.
- Accept direction and change in assignment.
- Include appropriate staff in decision-making processes.
- Complete duties as assigned.
- Sterilize instruments and equipment using the autoclave.
- Order and maintain medications and supplies to par level.
- Ensure exam rooms are appropriately cleaned and stocked.
- Collect and properly label specimens.
- Maintain specimen logbooks and report abnormal results.
- Greet and direct patients to exam room.
- Complete vitals and other clinical duties.
- Triage and room patients within 10 to 15 minutes of arrival.
- Assist providers in setting up and performing procedures.
Requirements
- High School/GED
- 1-3 years experience
- Effective English communication skills, written, oral and interpersonal.
- Ability to use basic data entry and retrieval system.
- Additional education required by specialty areas/patience care locations.
- OHS Annual Assessment
- OHS Fit Test
- NYS License for Practical Nurse and current registration to practice as an LPN in New York State
