About the Role

Responsible for providing individual and group therapy, psychological/social services, and discharge planning for an inpatient psychiatric unit. Provides counseling and aftercare mental health services, completes insurance reviews, and collaborates with physicians for safe discharges. May rotate weekend coverage for mental health evaluations in Emergency Departments.

Responsibilities

• Assessment, diagnosis, intervention, and treatment planning for pediatric, adolescent, adult, and geriatric patients.
• Performs initial behavioral health assessments on new patients.
• Develops mental health diagnoses using the DSM-5.
• Makes recommendations for age and illness-specific treatment interventions to the ED provider.
• Performs a discharge summary on all patients providing criteria.
• Provides referral to other behavioral health and/or medical providers as necessary.
• Develops discharge plans for services not performable within outpatient mental health.
• Coordinates referrals for assessment when symptom identification and behaviors indicate psychopharmacological treatment.
• Makes referral for interventions appropriate to age group and identified illness.
• Performs patient record documentation and maintenance.
• Contacts internal and external providers/agencies to gather medical record information.
• Attends and participates in Health System and/or community related activities.
• Assumes on-call duties for designated periods of time.
• Contacts insurance companies to obtain benefits and authorization for treatment and appeals if necessary.
• Completes concurrent insurance reviews to ensure reimbursement.
• Collaborates with physicians for safe discharges.
• Ensures proper services and education are in place for safe discharges.
• Completes required medical record documentation on a timely basis.
• Participates in the Quality Management Plan for inpatient/outpatient services.
• Completes necessary continuing education requirements.
• Works in a team environment.

Requirements

• Master’s Degree in Social Work from Accredited Graduate Program
• Strong organizational and communication skills
• NYS LMSW License within 6 months of hire date required
• Successful candidates might be required to undergo a background verification with an external vendor.
