About the Role
Helio Health is seeking a Licensed Practical Nurse (LPN) for its Forensic ACT program to provide comprehensive treatment for individuals affected by substance use, mental health, and other behavioral health disorders.
Responsibilities
- Provide medical and counseling services as part of a multi-disciplinary treatment team in various clinical settings.
- Deliver client services for medical issues.
- Monitor and document clients’ medical status and withdrawal symptoms, reporting to supervisors.
- Collaborate with medical staff for continuity of client care.
- Communicate client medical concerns with clinical staff.
- Interpret medical information and provide medical services directly to clients.
- Perform direct client care including medical assessments, implementing medical orders, and reporting communicable diseases.
- Implement public health and screening protocols.
- Provide and document medical consultations, assessments, examinations, and treatment planning.
Requirements
- Must hold a current New York State Licensed Practical Nurse (LPN) license.
- Possess at least one year of experience or equivalent training in integrated mental health and substance use services.
- Proficient in basic computer skills.
- Must possess a valid driver’s license and have personal transportation.
- Maintain minimum auto insurance coverage of $100,000/person and $300,000/accident for work-related driving.
- Provide proof of current auto insurance coverage and updated declaration pages as required.
