About the Role
Improve patient health through an innovative, patient-focused culture dedicated to making a difference in the communities served.
Responsibilities
- Assist with direct and indirect patient care under the supervision of an RN or LPN.
- Perform Activity of Daily Living (ADL) care, including hygiene, toileting, and changing bed linens.
- Maintain a decluttered patient care area and understand available patient care products.
- Implement safety and preventative care measures, including infection, skin, and fall prevention.
- Ensure patient safety by setting and responding to alarms, and maintaining environmental safety.
- Assist with patient mobility, including turning, offloading, ambulation, and using mobility assist devices and patient lift equipment.
- Assist with meal delivery, setup, and feeding, and document consumption.
- Perform patient data collection, including vital signs, EKG, bladder scan, and telemetry.
- Document patient information in EMR, including vital signs, height/weight, and intake/output.
- Perform essential care documentation, intentional rounding, and direct patient monitoring.
- Provide Safety Attendant (SA) services at bedside for self-harm prevention and utilize video surveillance.
- Perform patient care interventions as delegated, including phlebotomy, EKGs, specimen collection, and blood glucose measurement.
- Prepare patient rooms for admissions and discharges, update whiteboards, and stock supplies.
- Care for patients with invasive lines and devices within scope of practice.
- Transport patients on and off the unit.
- Respond to emergency situations.
- Communicate with the patient care team to ensure quality care delivery.
- Perform unit-specific or patient population-specific interventions as delegated.
- Communicate effectively with the nursing and healthcare team, reporting out-of-range data and patient changes.
- Perform shift reports with oncoming staff.
- Demonstrate initiative and understanding of job duties through communication and self-initiative.
- Relate to and respect each patient individually, meeting their cognitive, physical, emotional needs, and respecting preferences, cultural, and religious beliefs.
- Perform other duties as requested by the healthcare and leadership team within the scope of practice.
Requirements
- High School Diploma or GED preferred.
- Requires one year of direct patient care experience in an acute care hospital, long-term care facility, rehabilitation facility, or licensed home care facility/agency.
- OR Completion of one year of RN school (ADN or BSN program) with a minimum of 30 hours of clinical patient care.
- OR Certified Nursing Assistant (CNA) with a current license.
- OR EMT certification with less than 1 year of volunteer experience and current certification.
- BLS Certification required within 6 months from date of hire.
