About the Role
Supervises the delivery of care coordination services to an adult population with a history of substance abuse, including those with specialized needs such as chronic medical conditions, mental health issues, and veteran status. Assumes overall facility responsibility in the absence of the Site Director and Clinical Coordinator.
Responsibilities
- Provide clinical case-management supervision to case managers.
- Offer counseling and support to enhance clients' capacity to benefit from treatment.
- Perform crisis intervention to assist clients with sanctions and barriers to self-sufficiency.
- Engage in community outreach to locate and enroll appropriate clients.
- Provide assessment and support services for access to specialized services (Mental Health, Social Security Disability, Veteran’s Resources).
- Facilitate access to job training, job search, placement, and stabilization.
- Advocate for clients and report on client activity.
- Adhere to agency protocols for equipment maintenance and security.
- Participate as a member of the multi-disciplinary team and weekly case conferences.
- Ensure accuracy of treatment plans and clinical service delivery.
- Develop clinical programming tailored to the population's needs.
- Perform other assigned duties.
Requirements
- 2-3 years of experience in a substance abuse setting.
- Experience providing clinical supervision in a residential setting.
- Extensive knowledge of community resources for clients with substance use disorders, mental health disorders, chronic medical conditions, and veterans.
