About the Role
The Care Navigator is a core member of an Integrated Care team, responsible for coordinating and supporting Mental Health care within the clinic and managing referrals to internal and external services. This role may involve providing evidence-based treatments or collaborating with other Mental Health providers.
Responsibilities
- Provides direct care services to patients individually and in groups, including activities of daily living, social, and psychiatric rehabilitation per treatment plan.
- Participates in comprehensive treatment plan development by sharing patient needs and wants.
- Documents and communicates patient response to treatment effectively, accurately, and in a timely manner.
- Provides comprehensive care in a professional and caring manner, taking appropriate actions and communicating findings to the multidisciplinary team.
- Intercedes on behalf of customers to ensure access to required services.
- Participates in the peer review process and utilizes quality improvement techniques.
- Contributes as a member of the multidisciplinary team by providing feedback and maintaining professional relationships.
- Enhances professional growth and development through continuous learning and participation in educational programs.
- Understands and practices established RGHS policies, procedures, goals, standards of care, quality improvement activities, safety, and environmental and infection control.
Requirements
- Minimum of 2 years of experience in a Human Services related field, medical related field, customer service, or client/patient interfacing experience.
- Associate’s degree required.
- Bachelor’s degree preferred.
- Sedentary physical requirements: primarily office-based with prolonged sitting, minimal physical effort, and lifting under 10 lbs.
- Requires fine motor skills for computer work, writing, and telephone use.
- Requires clear visual and auditory ability.
- Cognitive and emotional demands include extended concentration, frequent interruptions, independent decision-making, and information processing.
- May require flexible hours, TB screening, or a valid driver’s license.
