About the Role

The FGP Clinical Nutritionist/Certified Diabetic Educator supports patients with diabetes, pre-diabetes, and gestational diabetes through interventions and education to achieve treatment goals.

Responsibilities

Develops insulin and oral hypoglycemic protocols for effective patient management.
Educates practice RNs to enhance understanding of clinical diabetes care.
Maintains optimum practice operations for diabetic patient care.
Participates in clinical co-management with physicians and care providers, modifying diabetes regimens based on patient factors and data.
Critically examines diabetes educator practice and performance to maintain high standards of care.
Completes all required documentation for diabetes patient care.
Ensures ADA certification standards are met and maintained.
Promotes diabetes self-management education and training (DSME/T) as integral to diabetes care.
Promotes documentation of DSME/T outcomes and encourages research to improve care.
Provides assessment, education, and therapeutic interventions for patients with diabetes, pre-diabetes, gestational diabetes, hypoglycemia, and those on insulin pumps or continuous glucose monitoring.
Develops and updates nutrition education manuals for outpatient settings.
Develops and implements medically supervised weight management programs.
Evaluates and provides individual and group education, including nutrition consultation with family members, staff, and physicians.
Documents consultations/education and provides updates to referring physicians.
Attends and participates in community lectures and support groups.
Provides education programs for marketing, community benefit, and staff education.
Attends multidisciplinary patient care rounds and relevant clinical meetings.
Prescribes food and nutrients, including nutritional support, per protocol or under MD direction.
Ensures patients' needs for special diets and altered meal schedules are met.
Identifies patient diet knowledge deficits.
Works with physicians on establishing provisional status as a Center of Excellence under ASMS guidelines.
Assists with program development and marketing.
Manages and requests appropriate educational resources.
Assists in tracking database outcomes and patient flow.
Involves nutrition care planning across the continuum of care, including servicing patient dietary staff, providing pre/postoperative classes, and follow-up care.
Implements performance improvement strategies identified through program outcome reviews.

Requirements

Bachelor's Degree in Nutrition, or equivalent education meeting CDRs requirement for certification.
Current Registered Dietitian Certification from the Commission on Dietetic Registration.
Certification from the National Certification Board for Diabetes Educators.
Minimum of 1 year of experience in diabetes education (hospital or outpatient).
Clinical competence in managing diabetic patients with oral agents, insulin, injectables (GLP-1 agonists, pramlintide), type 1, type 2, and gestational diabetes.
Proficiency in education and use of chronic subcutaneous insulin pumps.
Experience with use and interpretation of continuous glucose monitors and sensors.
Skill in management and modification of diabetic regimens based on glycemic control and care plan targets.
Basic computer skills including Microsoft Word, Windows, Excel, and email systems.
