About the Role
The Career Services Case Manager (CRED) provides case management and encourages participant engagement and retention for individuals involved in the justice system and those residing in communities impacted by violence. This role supports participants in navigating social and legal systems, mitigating risk factors, and accessing resources for stability and growth, focusing on career pathways, training, job placement, and economic mobility for Staten Island residents.
Responsibilities
- Devise and administer service needs assessment to identify participant needs.
- Work with Mental Health Professionals and referral agencies.
- Provide case management and encourage participant engagement and retention.
- Leverage knowledge of benefits and entitlements to assist participants in obtaining resources.
- Connect participants to internal and external agencies for support.
- Work with participants to identify and overcome barriers to program completion.
- Track participant progress towards goals.
- Escalate cases to the program mental health professional as appropriate.
- Collaborate with the broader CRED team for holistic participant support.
- Perform other tasks as assigned by the supervisor.
Requirements
- Demonstrated understanding of social service needs of under/unemployed adults (18-40) involved in the criminal justice system or impacted by community violence.
- Experience supporting case loads.
- Knowledge of social services, benefits, and entitlements.
- Case management experience.
- Cultural competence.
- Excellent communication skills.
- Experience working with City systems (public assistance, child welfare, education, housing).
