About the Role
The Nurse Practitioner will function as a Clinical Coordinator in the Post-Discharge Follow Up service, supporting the 1115 Medicaid Waiver. This role involves facilitating patient discharge, coordinating follow-up care, conducting patient visits, performing exams, diagnosing and treating conditions, prescribing medication, ordering tests, and providing preventative health services. The position also includes collaborating with staff for continuity of care and assisting with the Nurse Practitioner Fellowship Program.
Responsibilities
- Facilitate bedside touchpoints for patients slated for discharge.
- Direct Post-Discharge Coordinator and Scheduling Coordinator to maintain workflow and patient contact.
- Conduct virtual and in-person visits for post-discharge patients from WHMC inpatient and Emergency Room.
- Perform physical exams, evaluate patients, and document findings.
- Diagnose and treat diseases, provide education on discharge instructions, medical management, and follow-up appointments.
- Evaluate patients to determine the necessity of sooner follow-up.
- Prescribe medication in accordance with New York State and federal law.
- Order and interpret laboratory, radiological, and special examinations or tests.
- Provide preventative health services, including health education and counseling, to prevent 30-day readmission.
- Collaborate with clinicians and staff to ensure continuity of care and develop metrics for patient experience.
- Assist with the Primary Care Nurse Practitioner Fellowship Program, including teaching, curriculum development, and accreditation.
Requirements
- Master of Science degree in Nursing—Adult-Gerontology Primary Care Nurse Practitioner (preferred) or Family Nurse Practitioner, Doctor of Nursing Practice preferred.
- Completion of Primary Care Nurse Practitioner Fellowship Program.
- Graduate of a Nurse Practitioner education program registered by the New York State Education Department.
- Certification as an Adult-Gerontology Primary Nurse Practitioner or Family Nurse Practitioner from a New York State approved accrediting body – American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP).
- Current, unrestricted New York State Registered Nurse license and Nurse Practitioner license.
- Active Drug Enforcement Agency (DEA) registration or eligibility.
- Active Medicare/Medicaid Enrollment or eligibility.
- BLS or ACLS certification required.
- Bilingual Spanish (preferred).
Education Level
- Master of Science degree in Nursing—Adult-Gerontology Primary Care Nurse Practitioner (preferred) or Family Nurse Practitioner
- Doctor of Nursing Practice preferred
- Completion of Primary Care Nurse Practitioner Fellowship Program
- Graduate of a Nurse Practitioner education program registered by the New York State Education Department
