About the Role

The University of Rochester is seeking a patient referral specialist to manage department referrals, ensure data compliance, and improve patient scheduling and treatment coordination.

Responsibilities

  • Serve as the patient referral specialist with oversight of data and compliance.
  • Communicate with patients, families, and staff to identify barriers to appointment compliance and track assistance provided.
  • Manage all aspects of the referral process, impacting patient scheduling, treatment, care, and follow-up.
  • Adhere to approved protocols for working referrals.
  • Make decisions guided by protocols and practices requiring interpretation.
  • Maintain an expert-level understanding of the department/division.
  • Train new staff members as needed.
  • Manage department referrals and serve as a liaison between referring offices, specialists, and patients.
  • Coordinate scheduled visits and procedures using Epic Referral work queues.
  • Conduct data analyses to track patient compliance and monitor work queues.
  • Communicate with referring and referred-to departments to reconcile discrepancies.
  • Escalate case management when medical assessment is needed.
  • Prioritize referral requests using medical protocols and expedite urgent requests.
  • Request and coordinate team and patient meetings.
  • Participate as an active member of the care team.
  • Acquire insurance authorization for visits and testing, documenting information in Epic.
  • Document all communications pertaining to referrals and insurance authorization in Epic.
  • Perform needs assessments using the electronic medical record to ensure appropriate scheduling.
  • Capture accurate patient demographic and insurance information.
  • Adhere to RIM protocols for record verification.
  • Perform complex appointment scheduling and link referrals and ancillary services.
  • Provide patients with appointment and provider information, directions, and educational materials.
  • Provide regular data on patient compliance and strategies to improve it.
  • Ensure ancillary testing and specialty referrals are executed and results are received and acted upon.
  • Investigate, troubleshoot, and resolve issues related to information delivery/receipt.
  • Demonstrate expert medical knowledge to recognize urgent clinical situations.
  • Review complex referral requests and schedule with the appropriate provider.
  • Work with providers and clinical staff to establish patient care plans.
  • Serve as backup to the PSS scheduling team.
  • Process outgoing referrals and discuss care options with patients.
  • Ensure Meaningful Use requirements are met.
  • Transfer Summary of Care electronically via Epic or by facsimile/mail.
  • Process incoming referrals not generated within the UR system.
  • Complete referral entry for external referrals into Epic.
  • Coordinate ancillary testing and obtain outside records.
  • Perform other duties as assigned.

Requirements

  • High School diploma or equivalent and 2 years of relevant experience required.
  • Equivalent combination of education and experience.
  • Medical Terminology preferred.
  • Experience with surgical/appointment scheduling software preferred.
  • Experience with electronic medical records preferred.

Skills

  • Demonstrated customer relations skills required.
