About the Role
As a Health Home Care Manager, you will act as a leader in providing Health Home services to approximately 50-60 active clients, adhering to ACCESS Program Manual guidelines and NYSDOH requirements.
Responsibilities
- Be familiar with the ACCESS Program Manual, including Health Home Care Manager functions, COBRA, and Health Home reimbursable procedures.
- Complete screening, intake, and initial assessment, then develop and oversee individual-focused care plans with participants.
- Implement care plans with documented follow-up and monitoring.
- Provide advocacy and knowledge of available services for participants and their families.
- Conduct home visits as required, with a minimum of one per care plan period.
- Conduct regular case conferences with outside providers, ensuring at least one per client per care plan period.
- Be available for crisis intervention and remain with clients until issues are resolved.
- Complete accurate and up-to-date documentation and consents.
- Conduct daily follow-up to address issues, review activities, and discuss client insights.
- Maintain knowledge of HIV/AIDS symptoms, treatment, medications, substance abuse, and other chronic conditions.
- Foster a positive work environment, manage providers, delegate tasks, and address conflicts.
- Communicate effectively with supervisory personnel.
- Maintain monthly per member per month billing standards, meeting one of the 5 core Health Home functions.
- Complete HCBS Waiver Form if not meeting criteria for HCBS services.
- Complete all documentation related to HARP/HCBS services and assess clients for HCBS eligibility.
- Create individual-centered care plans integrating behavioral and physical health services, including HCBS for eligible clients.
- Coordinate with clients' Managed Care Organizations (MCOs) to ensure access to eligible HCBS services.
Requirements
- Minimum of one year of direct case management experience.
- Knowledge of HIV/AIDS, substance use/abuse, mental health, and other chronic illnesses.
- Meet physical and cognitive requirements for assigned tasks, with reasonable accommodations for disabilities.
- Ability to travel within the five boroughs of New York City via public transportation.
- Demonstrate proper judgment and decision-making skills.
- Excellent written, verbal, and organizational skills.
- Ability to manage time effectively.
- Ability to input encounters with clients and/or providers daily.
- Bilingual in Spanish preferred.
- Knowledge of computers/word processing is a plus.
