About the Role
The Nephrologist provides expert consultation and ongoing care for patients with renal conditions, including chronic kidney disease, acute kidney injury, and hypertension. This role involves leadership within a multidisciplinary team focused on delivering high-quality, patient-centered care aligned with Central Health’s mission to improve community health and well-being. Responsibilities may include supervising advanced practice providers and serving as an attending physician for learners. Preferred candidates will have experience in FQHC or community health settings, demonstrate cultural competence, and excel in preventive care and patient education. Familiarity with value-based care models, EHR systems, and telehealth is highly desirable, as is a commitment to addressing social determinants of health and reducing health disparities.
Responsibilities
- Examines patients, obtains medical histories, and treats acute and chronic illnesses.
- Orders, performs, and interprets diagnostic tests.
- Involves patients and their families in care and treatment decisions.
- Collaborates on improvements in care delivery and service with staff, other physicians, and management.
- Initiates appropriate specialty referrals.
- Provides clinical information to the interdisciplinary team clearly and timely.
- Ensures proper patient documentation is timely, accurate, and complete.
- Works with practice management to ensure compliance with billing, coding, and CPT code assignment.
- Achieves and maintains optimal patient access.
- Participates in the development of medical protocols, standard procedures, and management of patient-care-related activities.
- Gives and accepts feedback from all staff respectfully.
- Acts in a positive and professional manner, complying with practice policies, procedures, and federal and state laws.
- Performs other duties and responsibilities as assigned.
- Promotes health education in conjunction with other health professionals.
- Advises the Director of Specialty of any problems affecting the organization's quality and efficiency.
Requirements
- MD or DO from an accredited medical school
- Completion of a Residency in Internal Medicine
- Residency in Internal Medicine and Accredited Fellowship in Nephrology with Certification from the American Board of Medical Specialties.
- 2 years of clinical experience
- Unrestricted license to practice Cardiology (Medicine) in the State of Texas
- Board Certification by the American Board of Internal Medicine and American Board of Pulmonology
- Current Drug Enforcement Agency (DEA) number for the purpose of writing prescriptions
- Current Basic Life Support (BLS) Certification for Healthcare Providers from the American Heart Association or the American Red Cross
- Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) from the American Heart Association or the American Red Cross
