About the Role
Job Path supports people with developmental disabilities in making life choices and playing valued roles in their communities. The Community Connections program offers an alternative to group-oriented day programming, enabling individuals to find volunteer jobs, develop community connections, and receive continuing education and skill development support.
Responsibilities
- Identify, secure, and support volunteer work and other community activities for a caseload of individuals and their families.
- Build relationships with individuals and families to ensure they receive necessary services.
- Coordinate person-centered planning to identify interests, goals, and potential community activities.
- Oversee the implementation and modification of plans to ensure meaningful and enjoyable activities for individuals.
- Interview and hire staff to support individuals in achieving their goals.
- Coordinate and oversee virtual support services as needed.
- Provide assistance to virtual groups.
- Offer coverage, which may include face-to-face community presence or virtual support.
- Create and maintain support schedules.
- Supervise and mentor direct support professionals.
- Schedule and supervise direct support professionals responsible for implementing individual plans and activities.
- Provide training and ongoing supervision to staff on person-centered planning, values, and practices.
- Complete evaluations for Direct Support Professionals, focusing on competency areas and providing regular performance feedback.
- Assist staff in exploring neighborhoods and identifying volunteer jobs and community activities aligned with individual interests and needs.
- Assist staff with locating virtual activities suitable for individuals on the caseload.
- Approve staff timesheets bi-weekly.
- Approve Medicaid billing documentation daily.
- Complete all program billing and payroll documentation daily according to policy and procedures.
- Ensure compliance with internal and external quality assurance requirements.
Requirements
- Bachelor's Degree and at least two years of experience working with Autistic people or people with developmental disabilities.
- High school diploma or equivalent and six years of experience working with Autistic people or people with developmental disabilities.
- Ability to be flexible in their daily schedule.
- Displayed leadership skills.
- High sensitivity to the needs and preferences of individuals and their families.
- Excellent communication skills, both oral and written.
- Ability to maintain confidentiality at all times.
- Experience with community development and social work.
- Excellent human service planning skills and ability to engage a broad spectrum of community members in planning efforts.
- Knowledge of Google Workspace and Microsoft Suite.
