About the Role
This position provides fiscal, programmatic, and operational oversight of the Breakthrough II Apartment Treatment Program, a 45-bed OMH Licensed housing program assisting adults with serious mental illness in developing skills for independent living. The Supervisor collaborates with various BHSN departments to support the team and residents.
Responsibilities
- Oversees daily operations of the Apartment Program to ensure resident and program needs are met.
- Facilitates, develops, and assists with mentoring, onboarding, training, supervising, and monitoring Apartment Program staff and interns.
- Establishes and coordinates staff schedules to ensure adequate coverage for resident needs.
- Collaborates and communicates effectively with team members to ensure resident needs are met, including coordination of mental health/substance use/primary care services, funding, transportation, linkage to other services, care coordination, and discharge planning.
- Coordinates and communicates all aspects of admissions and discharges to the team.
- Possesses working knowledge of and adheres to 14 CRR NY-XIII 593 and 595 Office of Mental Health regulations and BHSN policies/procedures for residential programs.
- Participates in internal and/or external committees to inform and/or screen potential Breakthrough II OMH licensed housing applicants for eligibility and admission preference.
- Works collaboratively with the Residential Funding team, Finance Department, and outside agencies to ensure resident funding/insurance is in place and oversees program finances for fiscal responsibility.
- Direct oversight of Breakthrough II Administrative Assistant(s), Breakthrough II Peer Engagement Specialist(s), Breakthrough II Crisis Intervention Specialist, Senior Apartment Program Case Manager, Apartment Program Case Managers, and interns.
- Works collaboratively with internal and external entities to coordinate, communicate, and reinforce integrated residential and medical treatment.
- Ensures and monitors monthly site safety checks and resident fire drills are completed correctly and deficiencies are resolved promptly.
- Collaborates with participating landlords to resolve tenant/landlord disputes, maintain safe environments, and review/negotiate lease renewals.
- Collects and organizes various data throughout the year as requested.
- Participates, facilitates, and ensures accurate documentation of all staff meetings.
- Collaborates and coordinates with the Adult Residential Nursing team to ensure the implementation, integrity, and monitoring of the medical management program.
- Ensures regularly scheduled treatment team meetings are scheduled and team members participate and communicate outcomes effectively.
- Provides, monitors, and ensures participants receive restorative services as indicated on their service plan and accurate documentation.
- Monitors and reports outcomes/Quality Management activity.
- Observes resident self-administration of medication according to established protocols.
- Works with the Adult Residential Nursing Team to maintain accurate medication records, storage, handling, and monitoring.
- Assists residents in maintaining safe and sanitary dwellings and promptly reports facility/apartment maintenance issues.
- Adjusts schedule to meet program needs and ensure support services are provided.
- Complies with all BHSN and regulatory policies and procedures.
- Collaborates with the Compliance Department to ensure timely and accurate reporting of incidents.
- Provides and accurately documents direct support services with individuals in accordance with individualized service plans.
- Reviews and signs off as qualified mental health staff on all service plans.
- Attends various meetings with internal and external community partners.
- Performs additional duties as assigned within the scope of the job description.
Requirements
- Master’s and/or Bachelor’s degree in a human services-related field OR Associate’s degree in a human services-related field AND 3 years’ experience in human services OR High School Diploma AND 5 years’ experience in human services required.
- Case management and/or care coordination experience required.
- Experience with crisis resolution and strong de-escalation skills required.
- Strong interpersonal, communication, time management, problem-solving, leadership, supervisory, and organizational skills required.
- Supervisory management experience required.
- 3 years’ experience with vulnerable populations required; experience working with individuals recovering from severe mental illness and/or substance use disorders strongly preferred.
- Proficient in Microsoft Office software such as MS Word & Excel required.
- Reliable transportation to transition between varying program sites required.
- Must be able to lift up to 25 lbs and have the ability to climb and descend stairs.
- Ability to work a flexible schedule to accommodate program needs, including non-traditional business hours.
- Participate in lifting and moving resident’s belongings.
- Valid NYS driver’s license, acceptable to BHSN insurance carrier, for required local travel.
- CPR and NARCAN certification required within 60 days of employment.
