About the Role

Family Service League is seeking a full-time Case Manager for the Assertive Community Treatment (ACT) program. This role involves conducting home visits, providing education to clients, developing treatment plans, and connecting clients to treatment facilities.

Responsibilities

  • Complete Entitlement Applications.
  • Accompany clients to Entitlement Offices.
  • Provide money management and budgeting skills.
  • Ensure active entitlements, including Medicaid and/or other benefits.
  • Provide psycho-education to families and clients.
  • Facilitate groups.
  • Complete client assessments using relapse prevention plans.
  • Write treatment plans.
  • Provide motivational interviewing.
  • Connect clients to treatment facilities.
  • Correspond with managed care companies.
  • Apply for benefits for clients (i.e., Medicaid, SSI/SSD, HEAP, Food Stamps, etc.).
  • Provide crisis intervention.
  • Cover the crisis phone.
  • Participate in daily team meetings and case conferences.
  • Conduct daily home visits.
  • Perform all other duties as assigned.

Requirements

  • At least one year of experience with the severe and persistent mental illness (SPMI) population.
  • Familiarity with community resources, including DSS, SSI, Medicaid Managed Care and Housing.
  • Excellent organizational and interpersonal skills.
  • Strong communication skills, both verbal and written.
  • Ability to multi-task and prioritize in a constantly changing, fast-paced environment.
  • Proficient computer skills, including Microsoft Office.
  • Valid and clean New York State Driver's License.
  • Ability to travel to conduct home visits.
  • Ability to sit for long periods of time.
  • Ability to lift up to 10 lbs.
