About the Role
Provide personal care and supportive services to individuals in the community, in the privacy of their home. Ongoing opportunities to earn incentive pay for meeting performance metrics.
Responsibilities
- Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.
- Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with healthcare needs in his/her home.
- Immediately report changes in client’s condition to supervising professional.
- Ensure the safety of the client, client’s family and/or the employee.
- Maintain HIPAA.
- Complete and submit necessary patient documentation including charting tasks and observations on android device.
- Attend ongoing in-service programs in a timely manner, to maintain certification.
- Perform other duties as assigned.
Requirements
- Valid Driver's License and reliable transportation.
- Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program.
- Must have worked in a certified HHA or PCA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry).

Experience Level

Education Level
- High School diploma or GED preferred.
