About the Role
The Arc Chemung-Schuyler is seeking a compassionate and motivated Qualified Intellectual Disabilities Professional (QIDP) to support individuals with intellectual and developmental disabilities in achieving their goals, building independence, and living the lives they choose. This role combines case management, person-centered planning, service coordination, advocacy, and program oversight.
Responsibilities
- Coordinate and oversee services for individuals receiving residential and day supports, ensuring services are person-centered, compliant, and focused on positive outcomes.
- Help individuals build independence, achieve personal goals, strengthen community connections, and live the lives they choose.
- Coordinate and monitor person-centered services and supports.
- Facilitate and oversee Comprehensive Functional Assessments (CFAs) within Intermediate Care Facilities (ICFs).
- Collaborate with interdisciplinary teams to develop, implement, and monitor individualized service plans.
- Provide case management and service coordination for individuals receiving supports.
- Ensure compliance with regulatory requirements, Medicaid standards, and agency policies.
- Maintain accurate documentation, records, and reports.
- Monitor program quality and advocate for the needs and goals of individuals served.
- Provide leadership, guidance, and support to team members.
- Communicate effectively with individuals served, families, staff, and community providers.
- Identify service needs and implement proactive solutions to support positive outcomes.
Requirements
- Bachelor's Degree in Human Services or related field.
- Experience in developmental disabilities.
- Experience working in residential, day habilitation, or community-based support programs.
- Experience with person-centered planning and service coordination.
- Background in Social Work, Human Services, Psychology, or related disciplines.
- Compassionate and person-centered.

- Strong communicator and collaborator.
