About the Role
Humber River Health is seeking a Pharmacy Intern for Fall 2023 to provide drug therapy under the supervision of a Pharmacist, aiming to improve patient quality of life. This temporary full-time position offers an average of 5 days a week for 3 months, with preceptors provided to support the completion of internship requirements.
Responsibilities
- Provision of drug therapy, under supervision of a Pharmacist, for the purpose of achieving definite outcomes to improve patient’s quality of life.
- Clinical pharmacotherapy monitoring, drug information and adverse reaction reporting.
- Review of physician’s orders, computer entry of prescriptions in the MEDITECH system, creation and maintenance of patient medication profiles.
- Other “drug use control” functions which include checking of chemotherapy
- Quality assurance projects
- Best Possible Medication Histories and Medication Reconciliation
Requirements
- Completion of University of Toronto or University of Waterloo Undergraduate Pharmacy Program by June 2023
- Hospital pharmacy experience an asset.
- Demonstrated ability to work independently and as a member of a team.
- Proven ability to prioritize a consistent and varied workload.
- Good written and verbal communication skills required, in order to interact effectively with patients, peers, and other health care team members.
- Demonstrated ability to identify, prevent and resolve actual and/or potential drug related problems.
- Effective interpersonal skills.
- Excellent attendance and discipline free record required.
- An understanding of equity, diversity, and inclusion principles as they relate to health practices is an asset.
