About the Role

The Service Coordinator is a program-specific leadership role supporting the Inpatient Stroke unit in various aspects of its function. This role is responsible for overall patient flow, including planning, organizing, directing, coordinating, and evaluating activities related to patient admission and discharge. Additionally, it provides administrative and clinical leadership to staff, fostering an effective and efficient admission and discharge process.

Responsibilities

Leads and/or participates in developing and implementing organizational initiatives regarding quality improvements and program development.
Plans, organizes, directs, coordinates, and evaluates activities related to patient admission and discharge.
Ensures efficient and effective unit/program operation by providing mentorship and promoting a supportive learning environment for health professions, physicians, nurses, support and administrative staff.
Acts as a clinical resource for inter-professional teams by providing information based on clinical knowledge and theory/practice related to clients with complex health care needs.
Establishes and maintains a systematic review and assessment of department capacity.
Consults with and engages appropriate staff members, physicians and others to make independent decisions for patient admissions/discharges and team allocation.
Develops and maintains strong relationships with patients/families, external partners and other stakeholders by consulting, problem-solving cases and providing ongoing education.
May act as Manager’s delegate on clinical and practice issues of the program when the Program Manager is unavailable.

Requirements

Completion of a Master’s degree in a Health-related discipline or recognized equivalent required.
Current valid Certificate of Practice from a relevant Health-related College (i.e., OT, PT, RN, SLP, SW, RT, RD etc.) required.
Over 4 years up to and including 6 years practical and related experience and/or 2 years on-the-job training required.
Knowledge and experience in admission and discharge planning and case management required.
Knowledge and experience using utilization management principles and length of stay management strategies required.
Demonstrated leadership abilities and experience required.
Experience working with interdisciplinary teams is required.
Specialized experience with the rehabilitation stroke population is required.
Excellent decision making, organizational and time management skills required.
Knowledge of standards of practice and applicable healthcare legislation required.
Current Basic Life Support (C Level) Heart Saver plus (Canada) required.
Membership in relevant Ontario College of Health-related discipline required.
Current UHN employees must have successfully completed their probationary period, have a good employee record along with satisfactory attendance in accordance with UHN's attendance management program, to be eligible for consideration.
