About the Role
Supervise interventionists working collaboratively with youth in Rockland and Orange Counties with an intellectual or developmental disability who are at risk of separation from their families due to potential hospitalization or other out-of-home placement. This role also involves managing individual cases.
Responsibilities
- Supervise Home Behavior Crisis Interventionists
- Lead team huddles and staff meetings
- Determine appropriateness of referrals for the service
- Determine when cases are ready to close
- Manage approximately one case at a time
- Assist families in applying for OPWDD or OMH services
- Attend CSE meetings and other appointments with families as needed
Requirements
- Strong organization skills with accurate attention to detail
- Excellent communication skills
- Ability to navigate electronic health records
- Adaptability to a fast-paced environment
- Flexibility in adjusting work plans to meet family needs
- Valid driver's license
- Capability to move throughout the workday and accompany clients in the community
- Ability to move in tight spaces
