About the Role
This role involves supporting adults through safe and effective hospital discharge pathways, promoting independence, wellbeing, and positive outcomes within a fast-paced multidisciplinary environment.
Responsibilities
- Undertake Care Act 2014 assessments, reviews, and reassessments.
- Complete strengths-based, person-centred assessments and support planning.
- Manage a caseload of adults with learning disabilities and autism.
- Coordinate care and support services to meet assessed needs and promote independence.
- Work collaboratively with multidisciplinary teams, health partners, carers, and families.
- Advocate for service users' rights, choices, and wellbeing.
- Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
- Identify and respond to safeguarding concerns in line with statutory procedures.
- Assess and manage risk while promoting positive outcomes and independence.
- Maintain accurate, timely, and legally compliant case records and reports.
Requirements
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
