About the Role
The Pharmacist will provide patient-focused care at Toronto Rehabilitation Institute (TRI) and the UHN Reactivation Care Centre, focusing on improving patient outcomes through medication management and support of operational pharmacy activities.
Responsibilities
- Provides patient-focused care to improve patient outcomes by identifying, resolving, and preventing drug-related problems.
- Conducts thorough pharmacotherapy assessments, performs medication histories and medication reconciliation.
- Provides drug information to patients and healthcare teams.
- Supports operational patient care activities including facilitating drug distribution, access, procurement, and drug coverage.
- Promotes medication safety.
- Participates in education activities, including professional development, continuing education, mentorship, and training of new pharmacy staff and learners.
- Participates in the development, review, implementation, and evaluation of projects and initiatives aligned with departmental and organizational priorities.
- Provides drug information services to promote safe, effective, and cost-effective medication therapy.
- Effectively communicates with patients, colleagues, allied health professionals, and other members of the patient-care team.
- Performs other duties consistent with the job classification.
Requirements
- Possess a Bachelor of Science degree in Pharmacy, Doctor of Pharmacy degree or equivalent.
- Certificate of competency from the Pharmacy Examination Board of Canada.
- Active registration with the Ontario College of Pharmacists (Part A).
- Hospital pharmacy practice experience preferred.
- Clinical experience with patient-centered care in rehab is an asset.
- Hospital pharmacy residency certificate is an asset.
- Membership in the Canadian Society of Healthcare-Systems Pharmacy is an asset.
- Excellent verbal and written communication skills.
- Excellent interpersonal skills.
- Current UHN employees must have successfully completed their probationary period, have a good employee record along with satisfactory attendance in accordance with UHN's attendance management program, to be eligible for consideration.
